Work through the prompts using a fictional case or your own non-sensitive observations. This is a planning session, not a harder physical workout. Do not include names, medical records or genetic data in the workspace.

Use a fictional adult and state their goal, available time, equipment and experience. Keep clinical details out of this public template.

Establish scope

Write questions that determine whether your professional scope fits. Note when referral or additional guidance would be needed; the form itself does not clear anyone for exercise.

Draft the session

Select familiar movements and describe intended effort, rest, coaching cues and alternatives. Explain how the choices serve the stated goal.

Create the feedback loop

Decide what the member will report and how you will respond to difficulty or a missed session. Have an appropriately qualified professional review the plan before real use.
Make it work for you
Use the earlier levels whenever they fit better. Advanced levels add planning and judgment; they do not require harder exercise, restrictive eating or a higher health score.
